Online Requests

NHS App

You can create an account with the NHS app online; sign in on the NHS website or download the app on Android or iOS devices.

If you are using any third-party modules, such as the NHS app, to access your online account, you may have been asked to enter third party links to link your profile with our practice.

Third party links are also known as:

  • Organisation Code/ODS Code
  • Third-party Passphrase/Linkage Key
  • Account ID

Please visit the practice with a valid photo identification, such as a driver’s license or passport to request your third-party links.

We also offer an alternative to the NHS app called SystmOnline. If you want to create an account, please take a moment to read the information provided on the following page.

SystmOnline

You must visit the practice with a valid photo identification, such as a driver’s license or passport to create an online services account.

Once registered for the online services, you can access your account through an internet browser or the Airmid UK app. You can download this from the App Store on your iPhone or the Play Store on your Android phone.

You will be able to use the service to:

  • Order your repeat prescriptions
  • Make an appointment
  • Cancel an appointment
  • Change your contact details
  • View your records, medications and known allergies

After verifying your identity, we will provide you with an automatically generated username and password. Once you enter your password for the first time, you must reset it to something you will remember.

If you forget your password, visit the practice with your photo identification, and we will issue you a new password.

Before you sign up for this service, there are some key things to keep in mind.

  • You will need to verify your identity to access your medical records online.
  • You will be responsible for keeping your login details and password safe and secure.
  • If you suspect that someone else has accessed your record, change your password immediately or contact the practice for assistance.
  • Due to GDPR, we may not be able to offer online access to all parts of your medical record.
  • You may see information that you find upsetting, such as abnormal test results or bad news. If you need support or clarification, please contact the practice for assistance.
  • It’s up to you whether or not you share your medical information with family members or caregivers. However, it’s your responsibility to keep that information safe and secure.
  • If you feel pressured or coerced into sharing your medical information with someone else, it’s best not to register for access at this time.
  • If you notice any errors or information that does not pertain to you in your medical record, please log out immediately and contact the practice as soon as possible.
