Sick/Fit Note

You don’t need a sick/fit note if you have been off work for 7 days or less. You can self-certify by filling out a form provided by your employer.

If your employer doesn’t have a form, you can use the government SC2 self-certificate:

If your employer insists that you provide a sick/fit note during the self-certification period, we may be able to issue you with a private sick/fit note for a fee. You should discuss this with your employer to see if they will cover the cost.

Another reason to request a sick/fit note is if the DWP has asked you to provide one for your claim to benefit.

Note, that if your illness directly follows a hospital encounter, you must request a sick/fit note from the hospital. We can only issue a sick/fit note if you have consulted one of our clinicians or if we possess correspondence that allows the clinician to determine your fitness to work.
